Closed Wound Continuous Antibiotic Irrigation:
Two 24F Blake Drains
Vancomycin 1gm + Gentamicin 80mg in 1L NS at infusion rate 200mL/hr, connected to low wall suction.
How to order antibiotic irrigation solution in Epic:
1) Open patient, click "Manage Orders" Tab
2) Click "Active" Tab
3) Under "Manage Orders" on the right, type in "irrigation" and hit enter
4) In upper right of pop-up (order and order set search) click "Facility List"
5) Double Click NaCl 0.9% for irrigation soln and fill out the following:
6) dose: 1000mL
7) route: irrigation
8) frequency: continuous
9) rate: 200mL/hr
10) under Administration instructions, click the pink "***" box and delete it, freetype "1gm vancomycin and 80mg gentamicin"
11) click "add note to pharmacy" and type "tube to tube station 48"
12) call pharmacy to confirm order (vocera "non urgent medication request" or call 63202)
