Confirm discharge complete by opening patient station, find today's date and confirm status is "discharged"

Gather supplies:

Hospital gown

Hair cover and shoe cover

Belongings bag for clothes and personal items

Pillow + case


Patient Identification and Pre-Procedure Check

Confirm patient’s full name, allergies, and date of birth using two identifiers

Verify consent form is signed and scanned: "Implantable rhythm monitor (implant or explant)"


Patient Preparation

Walk patient to ACU bathroom to change


Instruct patent to:

Remove everything from the waist up

Put on a hospital gown open to the front

Place personal belongings in the bag


Transfer to Procedure Room

Walk patient to the procedure room once ready

Mark in-room in Epic


Prep and monitoring

Attach MacLab ECG leads, blood pressure cuff, pulse oximeter.

Ensure comfort and explain each step


During Procedure

Maintain patient comfort and safety

Monitor vital signs pre & post

Document in Procedure Log


Post-procedure Care

Remove ECG stickers, blood pressure cuff, and pulse oximeter

Assess the insertion site for bleeding or swelling

Ensure patient is stable before ambulation

Document no in-lab complications and complete sign out

Document Epic "out of room"


Print discharge instructions

Open "pre & post procedure" tab (may be in rarely used)

Click on "post-procedure"

Near bottom of list click on "discharge instructions", scroll up to confirm "Cardiac EP discharge instructions for loop implant/explant" (paraphrased) populated in top of window

Click "after visit summary" on left to print preview and change language on left if necessary

Accompany patient to the waiting room

Discharge patient from the facility
